Please read the following carefully. Submission of this Volunteer Application does not result automatically in approval. The undersigned (hereinafter referred to as “the volunteer”) hereby agrees to volunteer his or her services to Allie’s Haven Animal Rescue (AHAR) – a 501(c)(3) nonprofit organization. The volunteer acknowledges and understands the dogs involved in AHAR program may be untrained, unhealthy and/or unfriendly, and AHAR makes no representations whatsoever regarding the animal’s temperament, health (including the absence of diseases transmissible to humans or to other animals), ability, attitude, past experiences or trainability. The volunteer assumes all risks relating to working with the AHAR Rescue animals. The volunteer hereby releases AHAR, its officers, directors, and volunteers from any and all claims, actions, liabilities, damages, and costs of any kind arising out of volunteering for AHAR. If any member of the volunteer’s household or a visitor to the volunteer’s home or in the volunteer’s vehicle makes a claim, the volunteer will indemnify, defend, and hold AHAR and the AHAR officers, directors, and volunteers harmless from such claim. The volunteer agrees he/she/they is/are over eighteen years old or have included the signature of a parent or legal guardian assenting to volunteering with AHAR. The volunteer understands that if his/her/their volunteer application is accepted, he/she/they understand that he/she/they will be representing a non-profit organization when doing volunteer work for AHAR. The volunteer agrees that while carrying out any volunteer work for AHAR, he/she/they shall not engage in activities that are prohibited to a nonprofit, tax exempt organization. The volunteer shall not profit financially by any activities performed as a AHAR volunteer. The volunteer agrees not to represent AHAR in any type of political activity such as political initiatives or endorsing or condemning candidates for office. The volunteer understands that he/she/they may be held personally liable for any direct consequences of such misrepresentation. The volunteer hereby affirms that he/she/we are not currently under disciplinary action by the American Kennel Club, any national breed club, or any local kennel club, has/have not been convicted of any charges of cruelty, abuse, or neglect of animals, and am/are not engaged in large scale commercial breeding or sale of dogs. I/We agree to abide by the Articles of Incorporation and the Bylaws of Allie’s Haven Animal Rescue Bylaws.

I hereby agree to accept a volunteer position for Allie’s Haven Animal Rescue, Inc. (herein after referred to as AHAR), and in so doing, I agree to comply with all rules and regulations which may be established by AHAR, and I understand that failure to do so may result in my immediate termination as a volunteer. I further acknowledge that all services are performed at my own risk. On behalf of myself, my heirs, personal representatives and executors, I hereby release, discharge, indemnify and hold harmless AHAR, its board of directors, officers, staff and all other persons and organizations in any way connected with AHAR from any and all claims, causes of action, demands, of any nature or cause, including costs and attorney’s fees incurred by AHAR in connection with the same, based on damages or injuries which may be incurred or sustained by me in any way connected with my services for AHAR including but not limited to animal bites, accidents, or injuries.* Choose one: Agree
I understand that public relations is an important part of volunteering at AHAR on behalf of myself, my heirs, personal representatives, and executors, I allow AHAR to use any photographs taken of me for use in public relations efforts. Also, All photographs that I submit to AHAR will remain the property of the photographer.
